Dr. Rastogi will call all prescriptions for a patient as needed. Dr. Rastogi will also help her patient in recommending to the specialists as needed. The annual membership fees for joining Dr. Rastogi’s concierge medicine is as below:

Annual fees at the time of enrollment for a person is $720/year ($60/month). These membership fees are only for medical diagnosis and treatment under primary care. This will not include laser treatment and addiction treatment for a patient. This membership can not be transferred to another person or family member. The membership is nonrefundable and will continue for a year from the date of signing the contract and the full payment for the plan.
